Motor Carrier Schedules
- Schedule 1, Penalty Waiver Request, Declaration of State Assessment and Schedules 2 through 5 are now available for downloading and making a payment through the KSWebTax Taxcenter. You will need to register for an account before using KSWebTax for the first time. Click here to proceed to the KSWebTax website and register for your account.
- Schedules 2 through 5 and the Penalty Waiver Request can also be printed out and mailed in by clicking on the links below. NOTE: If you are having trouble with these forms or would like them in Excel (.xls) format, please send an email to pvd_mctax@kdor.state.ks.us.

- NOTE: Schedules 1's will be mailed as of March 7, 2008 and the filing deadline is April 11, 2008.
